A review into the death of five-year-old Kumanjayi Little Baby found that child protection authorities failed to adequately support her mother while she was experiencing domestic violence and trying to protect her children. Despite identifying systemic failures in safety assessments and multiple referrals to child protection services, the interim report was unable to establish a direct causal link between these failures and the child's alleged murder.
